Pet Prescription / Food Order Form
To ensure you are not left waiting at the time of collection it is advisable to order your food or medication 48 hours in advance. Prescriptions often need to be reviewed by the vet on duty.
Simply fill out the form selecting the relevant clinic where you want to pick up the medication or food. It is very important you fill in a contact number and where possible this is a mobile number. Please note that prescription medications can only be ordered if your pet has been seen by a vet in the past 6 months.
Please allow up to 48 hours for your orders to be completed.
